Balance statements and Payout Request Forms for the Fall 2013 semester will be mailed in late May to all eligible scholars. You can also download the Payout Request Form, here.
Payout forms will be due back to the CSF office by July 8, 2013. Checks will be mailed to colleges on July 17, 2013 in time for the fall semester. Any Payout Request Forms received between now and then will be held until July. Any Payout Request Forms received after July 8th will be held for payout in the Spring 2014 Semester.
Please note that payouts can only be made after verifying that the scholar is a full-time accepted and enrolled student at an accredited institution within the United States for the upcoming semester. Students must be enrolled in a bachelor's-degree-track (four-year) program.
