Carson Scholars Fund 2013-2014 Application Request Form

If you would like to be added to our distribution list for the 2013-2014 scholarship season, please complete this form. Please note the following:

• We are using an online application this year. Completion of this form will add you to our email distribution list to receive the online application instructions.
• If you are trying to request a hard copy, please send an email to Katie clearly stating that request. Include your school name and address in the request.
• Only accredited K-12 schools in the United States are eligible to participate. Students already in college or those living outside of the United States are not eligible for our program.
• Only school officials (teachers, counselors, principals) can request an application. If you are a parent or a student, do not complete this request form.
• In the "title" field below, please put your job title/role at your school.
